For anyone who is not in a lock down zone and attends Dixon Field to either participate or watch any activity run by Gisborne SC is required to fill in the applicable declaration form below.

The club’s committee recommends you you do not attend the clubs activities if you:

• Have tested positive to COVID-19

• Have been unwell or had any flu-like or respiratory symptoms (even if mild)

• Are living with a diagnosed person of Covid-19

• Have been in contact with a known or suspected case of COVID-19

• Are considered a high risk of contracting the virus

• Are not either a player, players parent/guardian or coach/team manager
